Ghost Guestbook Entries

I'm getting blank entries to a guestbook (http://www.josiahmackenzie.com/guestbook) I made, even though the "sign" page is JavaScript-validated and the corresponding MySQL entry fields are "NOT NULL". Does anyone know what could be causing this? It's driving me crazy! :angry:

is there any info that corresponds to the time those entries were made in your logs?

Where would I find that?

any field that autopopulates the current date or something of that nature...
